"FAQs You Should Know..."

My First Visit

Q: What do I need to bring to my first visit?
A:
Please bring your insurance card and bring the Health History form which you can download the Health History form by clicking on the link.  New patients should also be prepared to pay $50 that will be applied to charges not covered by insurance.

Q: What if my former dentist has my chart and/or x-rays?
A:
With your written authorization we are happy to request your chart and x-rays from your former dentist.

Q: What if I have a heart murmur, have had a knee or hip replacement, cosmetic surgery or other conditions that may require pre-medications?
A:
When you make your appointment please let us know if you have any of these conditions. We generally require that you take antibiotic medications before you arrive for your visit. We will be glad to contact your physician to determine whether you will need to be pre-medicated.

Insurance

Q: Do you accept insurance?
A:
Yes, we accept all dental insurance plans where the patient is allowed to select any provider of their choosing.

Q: Do you accept Preferred Provider Option (PPO) Plans?
A:
Yes, we accept most PPO Plans, including Blue Cross, Blue Shield, AETNA, MetLife, Delta Dental, Guardian and more.

Q: Do you accept Health or Dental Maintenance Organization (HMO or DMO) dental plans?
A:
No, we do not accept HMO or DMO plans.

Q:  Does your office bill the insurance company for me?
A:
For your convenience we gladly bill the insurance company for you.

Q:  Will your office seek pre-authorization from my insurance company before starting treatment?
A:
Yes. As a courtesy to our patients we go for pre-authorization so you will know if your insurance will cover the procedure, approximately how much your insurance will cover, and how much your portion is going to be.


Billing / Payments

Q: Do you accept payment arrangements?
A:
Yes, our staff will be glad to work out acceptable payment arrangements with our patients.

Q: What credit cards to you accept?
A:
We accept checks, cash, money orders, traveler's checks, VISA, Mastercard, and Carecredit.

Carecredit is a healthcare credit card specifically designed to pay for treatment and procedures not coverd by insurance.For more information, go to CareCredit.
  

Q: What form of payment do you accept?
A:
We accept checks, cash, money orders, traveler's checks, VISA, Mastercard, and Carecredit.

Q: Will you bill me for the services?
A:
Yes, we send our patients monthly statements.
